Why I Still Have a Gap Between My Teeth After 3 Years of Braces

Q

I have had braces for three years and I’m getting them off in May. Problem is, I have a huge gap in my teeth. My orthodontist pulled two of my teeth three years ago and I’ve complained that I don’t like walking around with a huge gap in my teeth. Are there any other options and how can I discuss this with my dentist?

A

After wearing braces for three years, those gaps should likely have closed. With May approaching, it’s reasonable to be concerned they won’t resolve by then.

My recommendation: see at least two other orthodontists while your braces are still on so they can evaluate the mechanics your current orthodontist is using to close the spaces.

Make sure at least one of the orthodontists you consult has expertise with Invisalign, as clear aligners may be a better option depending on how your teeth need to move.

Many cases that begin with extractions can be difficult to finish successfully because adjacent teeth often have too much distance to move. Instead of translating bodily to close the space, teeth sometimes tip or rotate, which can compromise function and the bite over time.

In many similar cases, using elastics (rubber bands) is critical to achieve proper space closure. If elastics are not worn as instructed, outcomes tend to be unsatisfactory. Extraction cases can also be associated with temporomandibular joint symptoms, such as clicking; bring this up with any orthodontist you consult if you have jaw concerns.

At this stage of treatment, it’s prudent to verify that the original treatment plan was appropriate. A second opinion can reveal alternative approaches and may allow you to alter the plan without starting from scratch. Replanning or “rebooting” treatment usually does not mean another full multi-year course; changes can often be implemented in a much shorter time frame.

Best of luck discussing your options and finding a treatment path that closes the gap while preserving a healthy bite.
